Case Manager

We are currently seeking a Case Manager for Hopewell’s community-based transition services program located in the Larchmere community in Cleveland, Ohio.

This position plays a vital role in supporting the success of Hopewell’s transitional services program in the greater Cleveland, Ohio area, with a focus on community integration and supporting clients in apartment-based, independent living settings. The Case Manager collaborates closely with Hopewell’s farm-based residential therapeutic community staff to ensure smooth, seamless transitions from farm to city, while providing ongoing, community-based support that promotes stability, independence, and long-term success. The Case Manager coordinates services both internally and with community partners to support effective transition planning and logistics. This role involves working with individuals and small groups to deliver targeted interventions that advance independent living goals, including skill development, community engagement, and connection to vocational and educational opportunities. The position requires periodic travel to Hopewell Farm in Middlefield, Ohio, to support continuity of care and successful transitions.

Experience Requirements:

  • 5 years of experience in case management or direct care role with individuals with serious mental illness.
  • Knowledge and a minimum of 3 years’ experience in processing the application for SSDI/SSI, Medicaid/Medicare Benefits, and other related areas of supplemental assistance.
  • Knowledge of local resources.
  • Training or experience working in a trauma informed care milieu.

Requirements:

  • High School Diploma/GED required.
  • A bachelor’s degree in a health and human services–related field (such as social work, psychology, or a related discipline) is preferred.
  • Must be 22 years of age or older.
  • Must pass a per-employment physical, two step TB test, drug test and background checks.
  • Valid Driver’s License

Job Type: Full time

Pay: $18-$24, based on experience.
